I seem to remember the discussion of very simple evaluation forms from a long time ago, but I can't locate any reference now.
As far as I can remember, the idea was a form separated into 3 parts. The top half is separated into two columns, one used to describe good qualities and aspects of someones behavior that could be improved (like a WWW/TALA). The bottom half is used to set goals for the future.
I would like to revisit this guidance if I can find it. Does anyone have an idea where to look?
