I have been listening to the review podcasts for several commutes and I had more than one question but I wanted to limit it to a few for now. What do you do if there is no internal HR document for reviews? Do I still use the SEER technique but attempt to develop my own template? Should I attempt to get my company to adopt a template? At this point I am thinking something is better than nothing.
Background:
Our reviews are based on anniversary date not calendar or fiscal year. We are a small company so we have one person that could be considered HR. When speaking with him I found that the review consists of taking the direct to lunch and telling them how they are doing. There is no established form or template to fill out and there is no documentation of the review in their file. I am doing O3's and getting better at feedback so I have compiled data for my first review. I did a few reviews last year but I knew I was missing something. This is a great opportunity for me to get better at the review process and to show my directs that I am really putting an effort into their review process.

Unless you want to work in HR, don't try to change the company policies or forms. Do yours well, and over time others will follow suit.
If you've been doing One-on-ones, providing feedback, and coaching your directs, the content of the review will flow naturally.
And it is the content, not the form, which is important. What have they accomplished? Where did they fall short? What skills do they need to develop next? In writing it up, you'll create your own 'template' by default.
Finally, make sure that the 3 R's are covered (as in the podcast). Doesn't need to be long - one page should be enough. Check this thread:
